Required Forms

New clients will be asked to complete both the Intake and Consent Form and the COVID-19 Policies and Consent Form prior to their session. The forms are a part of the registration process when booking for the first time.

Massage of Minors

Written parental consent is required for anyone under the age of 17 to receive a massage. The consent form can be downloaded by clicking the “Massage of Minor” button below. This form must be completed in addition to the Intake and Consent Form.

Breast Massage

For those clients needing lymphatic drainage or body contouring in the breast area, an additional breast massage consent form must be completed. Texas law requires written consent to perform a breast massage. This form must be completed in addition to the Intake and Consent Form.

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in my schedule that could have been filled by another client from the waitlist. As such, I require one business day’s notice (24 hours) for any cancellation or changes to your appointment. Monday appointments must be cancelled on the preceding Saturday by end of scheduled hours. Clients who provide less than one business day’s notice, or miss their appointment, will be charged for the appointment if I am unable to fill the time slot and will not be able to re-book until payment is received. If cancellations become frequent, the client will be asked to seek services elsewhere. Of course, I understand emergencies happen and will gladly accommodate any client if I can. Your health and well-being is always my first priority.

Lateness Policy

I request that you arrive early for your appointment in order for me to uphold our professional standards of being on time. Clients arriving late will be guaranteed only the remainder of their scheduled time, although the therapist will try to provide the full service time if possible. Clients arriving more than 15 minutes late may be rescheduled and subject to the cancellation policy fee. Please call if you are running late so arrangements can be discussed.

Sickness Policy

I recognize that both massage therapists and clients are vulnerable to infections and therefore ask clients to cancel appointments when they are feeling unwell. If a client has any of the following contagious illnesses the massage session will be rescheduled: Diarrhea, Vomiting, Fever, Chicken Pox, Measles, Mumps, Meningitis, Hepatitis A, Conjunctivitis, Rubella, Head Lice, Impetigo, Influenza, Meningococcal Disease, Polio, Ringworm, Scabies, Thrush, Whooping Cough and the common cold. Payment Policy

Payment in full is due at the end of the session unless otherwise arranged. Pre-paid sessions and gift certificates are also available.

Accepted payment methods are as follows: Personal check, exact cash, credit cards, and Venmo or Zelle electronic payment. Gratuity is appreciated but not required.

Draping

Draping, which is covering the body sections not being worked on with a sheet or towel, will be used in all instances.
